Financial and Insurance

In an effort to keep dental costs down while maintaining a high level of professional care, we have established the following payment plans for the use of our patients. Generally, full payment is required at the time of services.

Fees may be paid as follows:

  1. Prepayment, or payment at time of treatment with a 5% discount.
  2. Payment of balance in full within 10 days of the statement date.
  3. Use of your Visa, MasterCard, American Express and Discover.

Extended payments are available upon special request. Accounts outstanding more than 60 days from the treatment date will bear interest at 1.5% per month or 18% per annum. Long term financing is available through Care Credit. A discount is available for senior citizens age 65 and older.

All major treatment and treatment involving a laboratory procedure will require an appropriate down payment. Past due accounts will be charged a late fee of $5.

Insurance

If you have insurance we will help you determine the coverage you have available. We ask that you assign your insurance benefit to us. The balance is due in full within ten days of the date of the statement. Dr. Welden is a preferred provider of Blue Cross Blue Shield of Alabama and Southland. Dr. Ondocsin is a parcitipating provider for Delta Dental, Cigna and Met Life.

We accept CareCredit as a form of payment.

Professional care is provided to you, our patient, and not to an insurance company. Thus, the insurance company is responsible to the patient and the patient is responsible to the doctor. We will help in filing your claim and handling insurance questions on your behalf from our office.
